O R D E R:-
In pursuance of the orders issued in G.O.1st read above, orders were issued in the G.O. 2nd read above, regarding recovery of contribution towards Employees Health Scheme from the salary/pension of employees/pensioners eligible for Employees Health Scheme from December, 2013 payable in January 1st 2014. However, as the scheme was not implemented in the United State of Andhra Pradesh, the said recovery was stopped.
2.  In the G.O. 6th read above, after State Reorganisation, orders were issued duly modifying the Employees Health Scheme introduced in G.O.Ms.No.174, 175, 176, HM&FW (M2) Dept., dated 01.11.2013 and G.O.Ms.No.26, HM &FW (M2) Dept., dated 05.02.2014.Therein inter-alia, it was ordered that

i)  The deductions from the salary/pension of the employees/pensioners towards contribution under the scheme, shall be effect from the salary / pension pertaining to November, 2014 payable on 01.12.2014, Medical reimbursement under APIMA Rules, 1972 will not be allowed for the treatment undergone on and after 01.12.2014. Finance Department will issue instructions to all the drawing
and Disbursing Officers, Pension Payment Officers/Sub-Treasury Officers accordingly.
ii)  The benefits enumerated above shall be implemented within the estimated annual budget of Rs.220 Crores, which is derived from 40% contributions from employees /pensioners and 60% contribution by the Government
iii)  The Finance Department will issue separate orders out lining the procedure and for advancing funds to the Aarogyasri Health Care Trust from the monthly contributions based on the utilisation and settlement of claims.
iv)  The Finance Department will issue orders for reimbursement of expenditure incurred by Aarogyasri Trust towards claims payment and administrative expenditure under Employees Health Scheme between 01.11.2013 and 31.10.2014.
3.  Accordingly, Government hereby order that the contribution towards Employees Health Scheme shall be recovered from the Salary/Pension of Employees/Pensioners starting from November, 2014 payable on 1st December, 2014 onwards and reiterate the earlier orders in this regard.
4.  All the Drawing and Disbursing Officers/ pension Payment Officers are hereby instructed to recover the contributions from the Employees/Pensioners as ordered in the references first and second read above from the salaries/pensions of November, 2014 payable in December, 2014 onwards. The contribution shall be as under:
a)  Rs.90/- per month (for Slab A consisting of employees with Pay Grades from 1 to IV and Slab B consisting of employees with Pay Grades from V to XVII).
b)  Rs.120/- per month (for Slab C consisting of employees with Pay Grades from XVIII to XXXII).
c)  The Pay Grades for three Slabs under other Pay Scales, such as University Grants Commission Pay Scales, will be those equivalent to the corresponding Pay Grades under the State Government.
d)  The contribution for service pensioners or family pensioners will be according to the present Pay Grade of the post from which the pensioner retired from service.
If both of the spouses are Government employees or Service pensioners, contribution by any one of the spouses is sufficient. In such a case, the applicant shall give a declaration to the effect that the other spouse is a Government employee / Service pensioner, duly indicating the Employee Code/Pensioner Code of the other spouse. All the Drawing and Disbursing Officers are instructed to obtain an undertaking from the employees in the proforma annexed to this order with immediate effect.
The proportionate Government Contribution is as follows:
i)  Rs.135/- per month for Slab A and Slab B category of employees and pensioners.
ii)  Rs.180/- per month for Slab C category of employees and pensioners

ORDER:

1.    In the G.O. first read  above, that has been  amended in the G.Os., 4th  and 5th  read above, the then Government of pre organised Andhra Pradesh State launched “Employees Health Scheme” with effect from 05-12-2013, to provide cashless treatment to the employees, pensioners and their dependent family members in empanelled network hospitals for the listed therapies notified in G.O.Ms.No.176 HM&FW (M2) Dept dated 01-11-2013,  duly replacing the reimbursement of expenditure incurred for inpatient care subject to the provisions of APIMA Rules, 1972.

2.    Consequent on  reorganization of Andhra Pradesh, Government has reviewed the implementation of the Employees Health Scheme.    After careful consideration of the matter, Government  hereby issue the following orders in modification of the orders issued in the Government Orders first to fourth read above,  with reference to the “Employees Health Scheme”

A.    The Scheme, which was originally launched with effect from  05-12-2013, shall be implemented with all the following  modifications,  in a full-fledged manner, with immediate effect. Health cards to all the remaining beneficiaries shall be issued immediately.

B.  In the first phase the following categories of beneficiaries will be covered.
a) Serving employees:
(i)  All regular State Government Employees, as defined under the       
     Fundamental Rules (FR), including Provincialised work  charged               
     employees; and
(ii)  Provincialised employees of Local Bodies
b)     Retired employees:
(i)    All Service Pensioners;
(ii)    Family pensioners and;
(iii)       Re-employed service pensioners

The Employees / Pensioners health cards shall be seeded with Aadhar data, which shall be the basis for Biometric identification under Employees Health Scheme

ORDER:

In the G.O. first read above, as amended in the G.Os., 4th  and 5th read above, the then  Government of Andhra Pradesh launched the “Employees Health Scheme” (EHS) with effect from 05-12-2013, to provide cashless treatment to the employees, pensioners and their dependent family members in empanelled network hospitals for the listed therapies notified in G.O.Ms.No.176 HM&FW (M2) Dept dated 01-11-2013, and this scheme ipso facto replaces the existing reimbursement system under APIMA Rules, 1972. In para 4.2.2.1 of the G.O., first read above it is mentioned that Out Patient (O.P)  treatment for pre-defined long-term (Chronic) diseases will be provided in notified hospitals and that orders on the modalities and provision of budget to the notified hospitals will be issued   separately.  In his letter 6th read above, the Chief Executive Officer, Aarogyasri Healthcare Trust had submitted proposals for issuing guidelines on ‘chronic OP treatment’.

2.  Consequent on reorganization of Andhra Pradesh, Government have reviewed the implementation of the Employees Health Scheme and has decided that out patient treatment for chronic  diseases, including consultation, medicines and diagnostics shall be provided in the notified Government Hospitals and the expenditure on this would be met from within 15% of the total estimated annual budget of Rs.220 crores.

3.    Accordingly, after careful examination, Government hereby issue the following guidelines for Chronic Out-Patient treatment in notified Government Hospitals:

AP Govt NTR Arogya Seva Scheme

O R D E R:

In the G.O. first read above, the erstwhile Government of composite State of Andhra Pradesh launched a community health insurance scheme called “Rajiv Aarogyasri Health Insurance Scheme” with effect from 01.04.2007 with the objective of providing quality health care to the poor for identified diseases through a network of service providers from Government and private sector.

2. On consequent reorganization of the State of Andhra Pradesh into the States of Telangana and Andhra Pradesh with effect from 02-06-2014, orders were issued in the G.O. 5th read above restructuring the Board of Trustees as a body of only officials of both the States, with the Principal Secretaries to Government, HM&FW Department of the two States as Chairman and Co-Chairman, based on their seniority. These orders will be re-visited in due course.

3. In the meeting held on 06-08-2014, which was attended by the Minister for Health, Minister for Municipal Administration, Chief Secretary, Principal Secretary, HM&FW., Principal Secretary, Finance and Chief Executive Officer, Aarogyasri Healthcare Trust, the Chief Minister reviewed the implementation of ‘Aarogyasri Scheme’ and ‘Employees Health Scheme’.

4. Accordingly the Chief Executive Officer Aarogyasri Health Care Trust has submitted the proposals in the reference 7th read above with a request to Govt., to issue orders to rename the Rajiv Aarogyasri Scheme as “ Dr.Nandamuri Taraka Ramarao Aarogya Seva” and also enhancement of coverage by adding new procedures. After careful examination of the matter, Government hereby issue the following orders:-

(i) The Rajiv Aarogyasri Scheme being implemented in the State of Andhra Pradesh shall be renamed, and henceforth be called “ Dr.Nandamuri Taraka Rama Rao Aarogya Seva”.

(ii) The 100 procedures indicated in Annexure shall be added to the existing 938 procedures and the renamed “ Dr.Nandamuri Taraka Rama Rao Aarogya Seva” Scheme will provide cashless treatment in the empanelled Network hospitals for a total 1038 procedures.

(iii) The existing limit of financial coverage i.e., Rs.2.00 lakhs per family per annum is here by enhanced to Rs.2.50 lakhs per family per annum.

5. This order issues with the concurrence of Finance Department vide their U.O.No.26/A2/EBS.V/2014, dated.12-9-2014.

AP Government NTR Arogya Seva

AP State Government will be starting NTR Arogya Seva from 2nd October 2014 for cashless treatment at the network hospitals which spread across Andhra Pradesh state. New Government is formed in the Andhra Pradesh after Assembly Elections 2014. Telugu Desam Party has won majority seats and forming government on the 8th of June 2014. Earlier the Telugu Desam Party has promised to introduce NTR Health Cards for cashless treatment in network hospitals for all including Employees and Pensioners.

Rajiv Aarogya Sri was established by the congress government in 2004, and ever since, people who are on BPL (Below Poverty Line) and contain White Ration Cards getting free medical treatment in the referral hospitals established across all major towns/cities in Andhra Pradesh. Telugu Desam Party is going to add few more deceases and is likely to pay Rs2,50,000 under NTR Health Cards (Arogya Seva) program.

ORDER
In the G.O. first read above, orders were issued notifying the ‘Employees Health Scheme’ with the objective of providing cashless treatment to the employees and pensioners of the State Government and their dependent family members, which will ipso facto replace the existing medical reimbursement system under APIMA Rules, 1972, with additional benefits such as post – operative care and treatment of chronic diseases which do not require hospitalization and treatment in empanelled hospitals. In the G.O.2nd read above, the Final Therapy Prices for the various packages under the Scheme, were adopted and notified.
2. In his 3rd read above, the Chief Executive Officer, Aarogyasri Healthcare Trust, which is the implementing agency, has stated that the package prices adopted and notified in the G.O. 2nd read above include cashless service from the point of OP Registration of a patient till the expiry of 10 days after discharge as Inpatient with a 30 day warranty after discharge and therefore the prices are inclusive of pre-evaluation cost of OP cases which need to be screened before an IP case is identified. He has expressed the view that this aspect has to be specifically indicated in the G.O., first read above under “Hospital Payments’ so as to avoid ambiguity and scope for misinterpretation. He has, therefore, suggested that a clause be inserted in the above G.O., clearly defining the package so as to include pre-evaluation cost.
3. Government after careful examination of the matter, hereby issue the following amendment to G.O.Ms.No.174, HM & FW (M2) Dept., dated.01.11.2013
AMENDMENT
In para 5.2, of the G.O., under the heading ‘Hospital Payments’ the following shall be inserted as ‘d’:
d) “The package will also include pre-evaluation of patients, including consultation and investigations even in the cases which do not finally end up with admission for IP treatment. The network hospital shall not raise any claim for such pre-evaluation”
4. The Chief Executive Officer, Aarogyasri Health Care Trust shall take necessary action.
5. This order issues with the concurrence of the Finance (Expr.M&H-I) Department vide their U.O.No.34359/471/A1/Expr.M&H-I/13, dated.04.01.2014

O R D E R:
In the G.O.1st read above, Government have issued orders for implementation of Employees Health Scheme (EHS) which is intended to provide cashless treatment to the State Government Employees / Pensioners and their dependent family members in empanelled network hospitals which will ipso facto replace the existing medical reimbursement system under APIMA Rules 1972, with additional benefits such as post-operative care and treatment of chronic diseases which do not require hospitalization and treatment in empanelled hospitals.
2.  In the letter 2nd read above, the Chief Executive Officer, Aarogyasri Health Care Trust has stated that in para 4.3 of the G.O.Ms.No.174, HM & FW (M2) Department, dated.01.11.2013 under ‘Financial Coverage’, the following is mentioned:
a.  The employees, pensioners and their dependent family members will be eligible for a sum of Rs.2.00 lakhs (Rupees Two Lakhs only) per episode of illness with no limit on the number of episodes as existing in APIMA Rules, 1972. However, in the cases where the cost of treatment exceeds Rs.2.00 lakhs, cashless treatment will continue. No network hospital shall deny the treatment in such cases.
b.  The limit of Rs.2.00 lakhs will not apply in cases where predetermined package rates are above Rs.2.00 lakhs.
c.  Chief Executive Officer, Aarogyasri Healthcare Trust will settle the claims which exceed Rs.2.00 lakhs, following the standard claims settlement process after obtaining the approval of the Technical committee constituted by the Government.
3.  The Chief Executive Officer, Aarogyasri Health Care Trust has further stated that already 10,44,000 temporary health cards have been generated under the Scheme, in respect of the Employees / Pensioners and their family members who have already enrolled. As many as 396 beneficiaries have undergone surgeries so far. In order to meet any eventuality where the cost of treatment exceeds Rs.2.00 lakhs, it is necessary to constitute a Technical Committee in terms of para.4.3 (c) of G.O.Ms.No.174, HM & FW (M2) Dept.,
dated.01.11.2013
4.  The Chief Executive Officer, Aarogyasri Health Care Trust has requested the Government that a ‘Technical Committee’ with the following composition may be constituted in terms of para.4.3 (c) of G.O.Ms.No.174, HM & FW (M2) Dept., dated.01.11.2013.
1.  Director of Medical Education  -  Member
2. Commissioner, APVVP   -  Member
3.  Executive Officer (P&C), AHCT  -  Member-Convener
he has also stated that the panel doctors of the Trust, who are specialists in the respective fields, will assist the Committee in the process of examining and making recommendations in the above process.
5.  Government after careful examination hereby constitute a ‘Technical Committee’ with the following composition in terms of para.4.3 (c) of G.O.Ms.No.174, HM&FW (M2) Dept., dated.01.11.2013.
1.  Additional Director of Medical Education  -  Member
2.  Joint Commissioner, APVVP (Medical)   -  Member

Subject: Employees Health Scheme - Providing Cash Less Medical Treatment to the State Government Employees, Pensioners and their dependent family members-Certain Instructions-Regarding.

While enclosing copies of the references along with its enclosures all the District Educational Officers and the Regional Joint Directors of School Education in sate are here by informed that Government have issued orders in their G.O 1st cited to submit employees data (HMRS) through ONLINE Web-based application.
Hence all the District Educational Officers and the Regional Joint Directors of School Education in the state are requested to arrange to see that the staff who are working under their jurisdiction should submit their data/ applications to the concerned Drawing and Disbursing Officers along with the following documents in digital form along with the filled application for uploading the same for issue of Health Cards and they are requested to complete the exercise as per Government instructions before 24.12.2013 positively.
a) Copy of Service Register( pages 1 and 2 old service register or pages 4 and 5 of new register).
b) Digital copy of ICAO complaint photograph of the employee and of each beneficiary.
c) Copy of Aadhar Card or Aadhar Card enrolment receipt for each beneficiary.
d) For the beneficiaries who are below five years if Aadhar Cards or Aadhar Card enrolment receipts are not available, birth certificate issued by the appropriate authority may be furnished.
e) In the case of family member or dependent suffers any disability, the disability certificate issued by the component authority has to be submitted.
f) If (c), (d) and (e) documents above are not immediately available remaining data may be entered without awaiting the documents. In such cases, employees be instructed to submit them before January 31, 2014.
Further all the District Educational Officers and the Regional Joint Directors of School Education in sate should submit their feedback to the Deputy Director (Planning) O/o C&DSE, A.P., Hyderabad without fail and see that the exercise should be completed by 24 -12-2013.

Government has issued orders introducing the Employees’ Health Scheme (EHS) and issue of Health Cards to the employees, their family members and dependents, for cashless treatment at empanelled hospitals.
In order to implement the HRMS (Human Resources Management System) and issue of Health Cards, the Government has decided to collect the data of all Government Employees online through a Web-based application established by the Finance Department Portal. It has been decided to collect the data directly from all the Drawing and Disbursing Officers (DDOs) to obviate delay in the matter
In this context, the Government hereby direct all the Drawing and Disbursing Officers (DDO) in the state to furnish the data relating to all employees mentioned above through web-based application by logging in to the Finance Department portal at “https://fdp.cgg.gov.in” by using their existing log-in ID (DDO code) issued by the Director of Treasuries and Accounts (DTA) using the default password “dataentry”. The detailed instructions are given in the annexure to this order. The relevant forms and the User Manual are available in the website mentioned above. The helpline numbers are also indicated in the above mentioned website for the benefit of the DDOs and other employees
The DDOs shall complete the data entry on or before January 5th 2014. All DDO shall ensure that the required data-forms are printed and supplied to all employees and other functionaries for whom they are drawing and disbursement officers, including subordinate offices and staff.
The Government hereby instructs that the pay and allowances for the month of January 2014 of the employees who fail to submit their data to the DDOs by December 24th 2013 and the pay and allowances of DDOs who fail to upload the data by January 5th 2014, will be claimed only after they submit their data.

A Health Card is required for Employee Health Scheme (EHS) which is started for AP State Govt Employees and Pensioners. Government of Andhra Pradesh, has started this scheme on the 5.12.2013. AP State Govt Employees have to enroll for Health Card, which enable them to get cashless treatment in the network hospitals notified across all andhra pradesh. Step by step procedure for getting registered for EHS is as follows.
1.Employee have to log in to the official website www.ehf.gov.in with their Treasury ID and Password
2.Then they have to click on the registration tab
3.Then they have to click "Initiate Health Card/View Application"
4.After that, they have to enter their Aadhar Number, or Aadhar Enrollment Number
5.Then they have to click "retrieve details"
6.Then they have to enter personnal details like date of joining, address, pay, post details Etc..
7.After that, they need to click the save button to save the information which they entered. Enrollment application saved successfully message will then be displayed upon successfully saving.
8.After that, they have to enter their family members details.
9.If a family member is below 5 years of age, then, they have to upload date of birth certificate of that child
10.After adding family members (beneficiaries), they have to attach the required documents online. Then they have to submit the application form.

ORDER
In the reference first to third read above, orders were issued for implementation of Employees Health Scheme (EHS) by providing Cashless Medical Treatment to the State Government Employees, Pensioners and their dependents.
2. In continuation to the orders read above, the following orders are hereby issued regarding recovery of contribution towards EHS from the salary/pension of employees/pensioners eligible for EHS as indicated below starting from December 2013 payable in January 1st 2014 onwards; the source of finances for
implementation of the Employees Health Scheme and creation of Public Account
i) All Drawing and Disbursing Officers/Pension Payment Officers are hereby instructed to recover the contributions from the Employees/ Pensioners as ordered in the reference first read above from the
salaries/pensions of December 2013 payable in January, 2014 onwards.
The contribution is as under:
a) Rs.90/- per month (for Slab A consisting of employees with Pay Grades from I to IV and Slab B consisting of employees with Pay Grades from V to XVII).
b) Rs.120/- per month (for Slab C consisting of employees with Pay Grades from XVIII to XXXII).
c) The Pay Grades for three Slabs under other Pay Scales, such as University Grants Commission Pay Scales, will be those equivalent to the corresponding Pay Grades under the State Government.
d) The contribution for service pensioners or family pensioners will be according to the present Pay Grade of the post from which the pensioner retired from service.
(ii) If both of the spouses are Government employees or Service Pensioners, contribution by any one of the spouses is sufficient. In such a case, the applicant shall give a declaration to the effect that the other spouse is a Government employee / Service Pensioner, duly indicating the Employee Code/Pensioner Code of the other spouse. All the Drawing and Disbursing Officers are instructed to obtain an undertaking from the employees in the proforma annexed to this order with immediate effect
iii) (a) The proportionate Government Contribution is as follows:
i) Rs.135/- per month for Slab A and Slab B category of employees and pensioners.
ii) Rs.180/- per month for Slab C category of employees and pensioners.
(b) The Government contribution for Employees Health Scheme will be made under the following head of account:

SH(10) Government Contributions for Employees Health Scheme 320 Contributions
iv) The contribution of the employees and pensioners on one hand and Government contribution together shall be deposited under the following heads of account under Employees Health Scheme Account along the lines of Contributory Pension Fund.

iii) In respect of employees who are on long leave the contribution towards Employees Health Scheme shall be recovered from their leave salary.
vi) In respect of employees who are under suspension and those who are on leave without pay, the concerned employee shall contribute the amount to the EHS fund through a challan in the first week and furnish the same to the Drawing and Disbursing Officers of the last station they have worked. The services under Employees Health Scheme shall be suspended temporarily if they fail to comply with the above instructions

Udyogasri Scheme for AP State Govt Employees and Pensioners will commence from 5th of December 2013. AP State Govt Employees and Pensioners will get cashless treatment under this Udyogasri Scheme. They will get free treatment in 335 network hospitals which are covered by Aarogyasri Trust. They have to submit their Temporary/Permanent Health Card to get free treatment in the network hospitals for about 1835 Diseases and Treatments. They can get free treatment upto 2 Lakh rupees. Employees, Pensioners, Teachers Medical Bills will be paid by the Aarogyasri Trust. They dont need to pay any amount on joining in the network hospitals. There will be a 'Aarogya Mitra' in every network hospitals, that takes care of the people who joined by submitting an Health Card. AP State Govt will start the Udyogasri Scheme on experimental basis tomorrow that is on 01.12.2013. They have to register online at the official web portal www.ehf.gov.in
Gazetted and Non Gazetted Employees have to pay a premium of Rs120 and Rs90 respectively to avail free treatment. Semi Private Ward will be allotted to the employees who pay Rs90/month as premium and Private Ward will be allotted to the employees who pay Rs120 as premium per month.

Rajiv Aarogyasri Scheme (RAS) is being implemented by Government from 01.04.2007 to financially assist 233 lakh poor families to avail medical treatment for identified diseases covering 938 In-patient (IP) therapies and 125 follow-up therapies vide G.O. first read above. Government have also decided to implement a similar cashless scheme (Employees Health Scheme) for Government pensioners and employees with coverage envisaged in APIMA rules, 1972 vide G.O. 2nd read above.
Aarogyasri Healthcare Trust, after deliberations and in consultation with specialists, has identified additional IP therapies for inclusion in employees health scheme, in addition to Aarogyasri packages.
2. AP Speciality Hospitals Association (ASHA) and AP Private Hospitals & Nursing Homes Association (APNA) representing the private hospitals empanelled under Rajiv Aarogyasri Scheme, submitted representations to Government, seeking revision of the package rates for Rajiv Aarogyasri Scheme and also clarifications on certain other issues. In Memo. No. 21233 /M2/2012-3 HM&FW (M2) Dept dated 06-02-2013, Government have constituted a committee headed by Principal Finance Secretary and consisting of Principal Secretary to Government, Health Medical & Family Welfare Department, Commissioner, A.P Vaidhya Vidhana Parishad, Director of Medical Education, as members, Chief Executive Officer, Aarogyasri Health Care Trust as member-convenor, and two representatives each from ASHA and APNA as members for examining the grievances of ASHA and APNA. ORDER:
In the G.O. 2" read above, Operational Guidelines were issued for enrolment of beneficiaries, duly defining the role of the beneficiary, Drawing and Disbursing Officers, Director of Treasuries & Accounts, Commissioner of Civil Supplies, Chief Executive Officer, Aarogyasri Health Care Trust, Heads of Department and District Collectors.
2.In the G.O. 4th read above, revised orders were issued, in supersession of the orders issued in the G.O. first read above, notifying the 'Employees Health Scheme (EHS)'and defining its features.
3.In continuation and partial modification of the Operational Guidelines issued in the G.O. 2" read above, Government hereby issue the following guidelines for issue of Health Cards to the beneficiaries covered under the Employees Health Scheme (EHS):
ISSUE OF HEALTH CARDS
The employee or pensioner will submit online application along with the following documents as prescribed in G.O. 2" read above. The rates for submission of application at mee seva centres are prescribed in the G.O. 3rd read above.
copy of Service Register (pages 1 and 2 of old service register or pages 4 and 5 of new service register) in case of employee
digital copy of ICAO compliant photograph of each beneficiary, and
copy of Aadhaar enrolment receipt or Aadhaar card of each beneficiary
Permanent Health Cards
The following process will be adopted for issue of permanent Health Cards
a)The submitted applications will be scrutinised by Aarogyasri Health Care Trust (AHCT)
b)The scrutinised applications will be forwarded to the Drawing and Disbursing Officers (DDO) in the case of employees and the Sub-Treasury Officers (STO) in the Districts /Assistant Pension Payment Officers (APPO) in Hyderabad in case of pensioners, who will in turn verify the applications and approve or reject in case of discrepancies. The logins of DDOs or STO/APPOs may be reassigned to other functionaries in the district by the District Collector in case the DDOs or STO/APPOs are unable to handle the verification work.
c)The approved applications will be sent, online, for printing of Health Cards for each individual beneficiary.
d)The printed cards will be delivered to the respective Card Issue Centres (CIC) in the districts as per option given by the applicant. The beneficiaries will be notified through short message service (SMS) on their mobile phone once the permanent card is printed.
e)The entire applicant family will thereafter go to the designated CIC, give the individual beneficiary finger printsas acknowledgement and receive the permanent cards
The permanent Health Cards issued under EHS will bebiometric fingerprint based health cards, will carry the Aadhaar number/Aadhaar Enrolment number and will be issued in the name of the District Collector concerned.
The biometric Health Cards of all Employees and Pensioners who have submitted their applications with Aadhaar numbers and full details will be issued health cards through the respective CICs within 30 days of submitting their complete application.
The Card Issue Centres (CIC) will be located at the rate of one in each Revenue Division and the District Collector will decide the location of CIC in each Revenue Division.
The work of issue of permanent biometric Health Cards at CICs is an ongoing process to be carried out by Aarogyasri Health Care Trust till all the beneficiaries are issued permanent health cards
